Hybrid Implants

Hybrid Implants were designed as the result of observations of patients with different implants for the last 35 years and longer. In 1965 first patients were treated with modern implants, From that time the efforts were made to increase bone to implant contact, implant stability (measured in ISQ) and shorten healing time to restore missing teeth as soon as possible. The first implants had moderately rough surface – just as they came from the milling machine and therefore called – ‘machined surface’. Some studies indicated that more rough surface grows bone around implants – faster and therefore from the year 2000 most manufacturers started to make rough implants only. At that time already some doctors ( Dr. Dennis Tarnow in 1993 ) indicated that the rough surface at the implant upper half may create a risk of bacterial accumulation in the case when the implant gets exposed. However these remarks were dismissed

Unfortunately around the year 2005-7 many clinicians started to observe progressive bone loss around these new rough implants once they got exposed. They haven’t seen that with the old ‘machined’ implants before, it was new to them and they called it: ‘PERI-IMPLANTITIS’ as an inflammation around an implant which may lead to implant loss. Further studies revealed extensive bacterial accumulation (biofilm) on the rough surface of the implant.

Because Peri-implantitis was not observed with old machined implants – the idea of a HYBRID IMPLANTS that are machined, not rough at its top half – came into life introduced by Prof. Massimo Simion from Milano University. These implants combine faster integration at the implant apex deep in the bone and safety of machined surface at the top where bacteria may accumulate.

Prevention of Peri-implantitis

The first step to save implants from Peri-implantitis is to use implants that have machined surface at the upper part like Hybrid Implants. The next steps are: proper daily hygiene around implants and regular control visits to examine condition of the implants and the bone around them.

Optimalization of the healing process

Rough surface applied to the lower part of the Hybrid Implant may still encourage faster bone formation there and thus greater implant stability to support crowns and bridges.

Evolution

IRES Hybrid Implants can also be coated with a double atomic layer of Hyaluronic Acid which is widely used in regenerative medicine. This Hyaluronic Acid layer, in laboratory studies, has demonstrated to further increase bone to implant contact by 2 – 3 times in comparison to implants not coated with Hyaluronic Acid.
